    Please note: By completing this form you agree to the following;

    Medical Release: The medical/mental health history listed is correct so far as I know, and the person herein is capable of engaging in all School for the Arts program activities (unless listed otherwise). I understand that the OFJCC assumes no financial obligation for emergency medical treatment.
    During an emergency I hereby give permission to the physician selected by the OFJCC to order X-rays, routine tests, injections, anesthesia, surgery, secure proper treatment, hospitalize and administer additional emergency treatment as needed for the listed student on this form.

    I agree to and understand the following guidelines: Participants agree to abide by the following rules and regulations set by the OFJCC for the health, safety and welfare of participants and instructors. Participants are not allowed to smoke, chew tobacco, possess any smoking materials, alcohol, illegal drugs, firecrackers or explosives, weapons, use lewd conduct, or inappropriate touching of any kind. Willful destruction to property will be the financial responsibility of the student or student’s parent/guardian. Children may not leave the property or established boundaries without OFJCC staff or parent/guardian permission. A parent or authorized guardian must sign out children under the age of 13. Children ages 13-17 may sign themselves out if a parent/guardian has sent the school coordinator notice of self-sign-out.

    The Oshman family Jewish Community center reserves the right and will to dismiss anyone from the program who violates this agreement. The School for the Arts Coordinator and Department Director reserve the right to determine what constitutes a violation of these rules and will enforce them as necessary.
